Immunization Information
Proof of Immunization
- original immunization card;
- blood titer lab report;
- dates of vaccination on the doctor's letterhead (stamped and signed by the doctor), ;
- official veteran's medical record
- high school/secondary immunization record (stamped and signed by the school)
Measles
The first dose should be administered on or after your first birthday and the second dose must be administered at least 28 days after the first dose. A blood titer lab report showing immunity to measles would also satisfy this requirement.
Proof of vaccination must be submitted on doctor's letterhead or immunization card. Students who have had measles disease must provide proof on doctor's letterhead or a blood titer lab report showing immunity. Vaccinations given before 1968 are not acceptable.
Mumps
Proof of vaccination must be submitted on doctor's letterhead or immunization card. Students who have had mumps disease must provide proof on doctor's letterhead or a blood titer lab report showing immunity. Vaccinations given before 1968 are not acceptable.
Rubella
One (1) dose of live rubella virus vaccine given after 1968 and on or after your first birthday.
Proof of vaccination must be submitted on doctor's letterhead or immunization card. Students who have had rubella disease must provide proof on doctor's letterhead or a blood titer lab report showing immunity. Vaccinations given before 1969 are not acceptable.
Physical Examination
In an effort to secure a baseline health status for each incoming student, a physical examination is requested. Students in the health disciplines MUST submit a physical examination using York College Medical Record / Physical Examination form available in the FORMS section of our webpage. The Physical Examination part of the form must be fully completed, stamped and signed by a licensed physician.
Immunization Regulation
Meningitis
New York State Public Health Law 2167 requires that all students complete a Meningococcal Meningitis Vaccination Response Form which indicates that they either 1) received the vaccine with proof of date 2) will receive the vaccine within 30 days or 3) refuse the vaccine since it is optional.
