Change of Major Minor form

The completion of this form does not guarantee admission to the following programs unless approval is granted by the program director; Health Education, Physical Education, Teacher Education programs 1 thru 6 and 7 thru 12, Nursing, Occupational Therapy, Physician Assistant and Social Work. A description of each program and criteria for admission can be found in the bulletin under the program title.
