To expedite your choice to enroll, you must have the following documents available:
- Birth Certificate or Passport (For you and/or your children)
- Marriage Certificate
- Social Security Card
- Green Card or Citizenship Papers
- Proof of Address (Utility or Credit Card Bill, Drivers License)
