  1. Your 2010 Income Tax Return & W-2 form
  2. Parents 2010 Income Tax Return & W-2 form
  3. Your PIN # (apply at www.pin.ed.gov)
  4. Alien Registration Card (if you are not a citizen)
  5. Parents' Social Security Number
  6. Parents' Date of Birth
  7. Your Social Security Number
  8. Driver's License
  9. Date your parents' were married, divorced, or separated
  10. 2011-12 FAFSA Worksheet (click here) must be completed before attending workshop
